Fixed-term Residence Permit for non-EU and non-EFTA nationals

Documents required:

  1. application form for a fixed-term residence permit explaining the grounds for residence in Poland for more than 90 days. Write legibly and provide detailed explanation. Submit the form in quadruplicate;
  2. 4 (four) color photographs taken during the last 6 months, sized 3,5cm x 4,5cm;
  3. valid travel document (passport) – have it with you for verification or present a certified copy. In special cases, when a foreign national does not have a valid travel document and has no means of obtaining one, other documents verifying identity or their certified copies are accepted;
  4. copy of all pages of a travel document (passport) in duplicate;
  5. confirmation of admission or continuation of study from a public or private college or university. Where :

    Required documents:
    •    a form confirming your admission to or continuation of study in Poland. The form is available in Lublin’s Voivodeship Office (ul. Spokojna 4, Room 50) or can be downloaded here. NOTE: Other forms or confirmations are not accepted.
    •    you should also request a separate confirmation on your school’s form stating that you are a student or that you have been admitted into a program.
    Time taken: 10 to 60 minutes
  6. documents proving that you have sufficient funds to support yourself during your entire stay and return home. You must have sufficient funds to support yourself and your family without receiving social welfare benefits;
  7. documents proving that you have sufficient funds to cover education-related expenses such as university fees;
  8. proof of payment of admission/continuation fees, if required by your institution of higher education;
  9. proof of health insurance or a document confirming you are entitled to use public health care services as a result of international agreements. You have a choice between the public Narodowy Fundusz Zdrowia (NFZ) [National Health Fund] and private health care providers.

    You need to allow at least 30 minutes to sign an agreement so arrive accordingly.
    Required documents:
    •    form “WNIOSEK O OBJĘCIE DOBROWOLNYM UBEZPIECZENIEM ZDROWOTNYM”  [Application for health insurance coverage]; available in Room 6 or online download here ;
    •    confirmation of residence registration (see Point 1 above) (you will keep the original)
    •    passport;
    •    confirmation of your student status from your Dean’s Office – university form;
    •    temporary residence card, PESEL number, and NIP number – if you already have them.

    After your agreement has been prepared, you need to pay your first premium of 50.30 PLN (46.80 + 4.50) at the nearest post office (there is one 150 meters from the NFZ). After you return to the NFZ and show proof of payment, you will sign the agreement and receive your copy.

  10. ZAS-W form confirming you have no outstanding tax obligations – available from your local Revenue Office

    •    Window 14 – submit the completed NIP-3 form available from Window 14 or online here;
    •    Window 5 – submit the completed request for a statement on tax arrears – form available in Window 5 or online here;

    Ask where you need to pay the fee (21 PLN) and when you can collect your statement (most often on the following day). NOTE: The cashier’s window is open 8:00 to 13:00.

  11. Confirmation of residence registration from the Resident Affairs Office;
  12. Proof of payment of the stamp duty of 340.00 PLN for a fixed-term residence permit.
    Note:
    •    application forms must be completed in Polish only;
    •    all documents in other languages accompanying the application must be translated into Polish by a sworn translator;
    •    foreign nationals residing in Poland must re-apply for temporary residence at least 45 days before the expiry of their visa or their current temporary residence permit.
