Residence registration for EU nationals

Documents required:

  1. the application form to register residence should be completed in Polish. You should submit the original form and 3 copies;

  3. confirmation of admission from a public or private college or university

Dean's Office of your school/college/faculty
Required documents:

  • a form confirming your admission to or continuation of study in Poland. The form is available in Lublin’s Voivodeship Office (ul. Spokojna 4, Room 50) or can be download here

NOTE: Other forms or confirmations are not accepted. You should also request a separate confirmation on your school’s form stating that you are a student or that you have been admitted into a program. Time taken: 10 to 60 minutes

Important:
different statements are not acceptable. Ask for individual statement from the Dean's office confirming the fact that you are a student or that you are going to continue your education in Poland.

4. proof of health insurance or a document confirming you are entitled to use public health care services as a result of international agreements. You have a choice between the public Narodowy Fundusz Zdrowia (NFZ) [National Health Fund] and private health care providers.

You need to allow at least 30 minutes to sign an agreement, so arrive accordingly. see the map

Required documents:

  • form “WNIOSEK O OBJĘCIE DOBROWOLNYM UBEZPIECZENIEM ZDROWOTNYM”  [Application for health insurance coverage]; available in Room 6 or online download here ;
  • confirmation of residence registration (see Point 1 above) (you will keep the original);
  • passport;
  • confirmation of your student status from your Dean’s Office – university form;
  • temporary residence card, PESEL number, and NIP number – if you already have them.

After your agreement has been prepared, you need to pay your first premium of 50.30 PLN (46.80 + 4.50) at the nearest post office (there is one 150 meters from the NFZ). After you return to the NFZ and show proof of payment, you will sign the agreement and receive your copy.

5. documents confirming that you have sufficient funds to support yourself and your family, if applicable, without receiving social welfare benefits, such as a written declaration of the above, a credit card, a signed and stamped bank statement issued no earlier than a month before your application to register your residence.
You need to show your travel document when applying. EU nationals can use other forms of identification confirming their identity and citizenship.
When you receive confirmation of your residence, you need to show your travel document or another form of identification confirming your identity and citizenship as well as proof of payment of 1 PLN .

Residence registration for family members who are EU nationals

If your family member is an EU national staying with you in Poland, registration procedures are simplified.

Documents required:

  1. the application form to register residence should be completed in Polish. You should submit the original form and 3 copies;
  2. document confirming your registration of temporary address;
  3. confirmation of residence registration issued to the EU national whose family is applying for residence registration

Other documents:

  • for a spouse – a copy of the marriage certificate or contract;
  • for children and grandchildren – a document confirming family ties and a document confirming the age or financial support by a EU national;
  • for parents and grandparents – a document confirming family ties and a document confirming the age or financial support by an EU national.

A valid travel document (passport) is required when submitting an application for residence registration for EU nationals’ family members. Other valid forms of identification confirming identity and citizenship are accepted. All documents in other languages accompanying the application must be translated into Polish by a sworn translator. The fee for the statement of the residence registration is 1 PLN.
